Dry Eyes Treatment

Dry eye disease is a multifactorial ocular-surface disorder defined by loss of tear-film homeostasis. Tear instability, hyperosmolarity, inflammation, surface damage and neurosensory abnormalities may contribute in different combinations.

Complementary-care boundary: Netra Restoration Therapy (NRT) does not replace examination by an optometrist or ophthalmologist, prescribed drops, punctal procedures, meibomian-gland treatment or evaluation for autoimmune disease and corneal neuropathic pain.

Dry eye is not one condition

Aqueous-deficient dry eye involves inadequate lacrimal tear production. Evaporative dry eye commonly involves meibomian-gland dysfunction and excessive evaporation. Many people have a mixed pattern. Ocular allergy, blepharitis, contact lenses, medications, screen behavior, hormonal changes, surgery, systemic autoimmune disease and environmental exposure may also contribute.

Common symptoms

  • Burning, stinging, grittiness or a foreign-body sensation
  • Fluctuating blur that may improve after blinking
  • Redness, irritation or excessive reflex tearing
  • Light sensitivity and reduced tolerance for screens, wind or air conditioning
  • Contact-lens intolerance
  • Pain that seems disproportionate to surface staining, which can suggest a neurosensory component

Condition-specific mechanisms

Tear-film instability and hyperosmolarity

Rapid tear breakup exposes the ocular surface and can concentrate tear solutes. Hyperosmolar stress promotes epithelial injury and inflammatory signaling, reinforcing the dry-eye cycle.

Meibomian-gland dysfunction

Altered meibum quality or obstructed glands reduce the tear film’s lipid layer, increasing evaporation. Lid-margin inflammation and incomplete blinking may worsen the problem.

Ocular-surface inflammation

Innate and adaptive immune pathways can maintain epithelial and lacrimal-unit dysfunction. This is why anti-inflammatory or immunomodulatory therapy is appropriate for selected patients.

Corneal nerve dysfunction

Corneal nerves support blink and tear reflexes and also transmit sensation. Reduced sensitivity, hypersensitivity, peripheral nerve injury or central sensitization may help explain why signs and symptoms do not always match.

Evaluation and differential diagnosis

A dry-eye evaluation may include symptom history, medication review, slit-lamp examination, tear breakup time, corneal and conjunctival staining, tear-volume testing, eyelid and meibomian-gland assessment, osmolarity or inflammatory markers, and corneal sensitivity testing. Clinicians may also consider allergy, recurrent corneal erosion, infection, exposure, conjunctivochalasis, neuropathic pain and Sjögren disease.

Standard care

Care depends on the driver and severity. Options may include preservative-free lubricants, gels or ointments; warm compresses and lid hygiene; environmental and screen modifications; prescription anti-inflammatory or immunomodulatory drops; short supervised corticosteroid courses; punctal occlusion; thermal pulsation or other meibomian-gland procedures; autologous serum tears; scleral lenses; and systemic evaluation when autoimmune disease is suspected.

How NRT may support dry-eye care

NRT may be used as an adjunctive, whole-person program focused on autonomic regulation, inflammatory balance, ocular-surface comfort and systemic contributors. Acupuncture studies and meta-analyses have reported possible improvements in symptoms and selected tear measures, but study quality and protocols vary. These findings do not justify replacing established ocular-surface treatment.

Frequently asked questions

Why can my eyes water when they are dry?

An unstable or irritated ocular surface can trigger reflex tearing. These tears may not restore the balanced lipid, aqueous and mucin components needed for durable comfort.

Is severe burning always caused by dryness?

No. Persistent burning, light-triggered pain or pain out of proportion to staining may require evaluation for corneal neuropathic pain and other ocular or neurologic causes.

Are all artificial tears equivalent?

No. Formulations differ in preservatives, viscosity, lipid content and intended use. A clinician can help select an option based on the dry-eye subtype and ocular surface.
