Still Losing Vision? When Standard Eye Treatment Isn't Enough

Evidence boundary: NRT is complementary care. It has not been established as a replacement for ophthalmic medication, injections, laser treatment or surgery, and outcomes cannot be generalized across different diseases or stages.

Effectiveness must be condition-specific

Glaucoma, diabetic retinal disease, inherited retinal degeneration, macular disease and corneal neuropathic pain do not share one outcome measure. A meaningful evaluation begins with the diagnosis, baseline function, disease stage, remaining viable tissue, conventional treatment status and a predefined measurement plan.

How outcomes may be measured

  • Visual acuity: standardized distance or near acuity under comparable testing conditions
  • Visual field: repeatable change on the same perimeter and test strategy, interpreted for reliability and learning effects
  • Contrast, color and low-luminance function: selected when relevant to the diagnosis
  • Retinal or corneal imaging: OCT, fundus imaging, topography or other tests performed by the treating eye specialist
  • Patient-reported function: reading, mobility, glare, light sensitivity, pain and activities of daily living using consistent questions

What counts as credible evidence?

Individual clinical observation

A before-and-after change in one patient can guide that patient’s care, but it cannot establish effectiveness for everyone. Regression to the mean, testing variability, concurrent treatment and natural fluctuation must be considered.

Prospective outcome collection

Predefined eligibility criteria, standardized testing, documented adverse events and complete follow-up provide stronger evidence than selectively reported success stories.

Controlled research

Comparison groups, masking where feasible, adequate sample size and independent statistical analysis are needed to determine whether an intervention caused an observed difference.

Why a seven-day response cannot be promised

Some patients may report early functional or comfort changes, while others may not. A fixed timeline cannot apply equally to chronic optic neuropathy, inherited photoreceptor disease, macular edema and corneal pain. Statements about response should therefore be presented as individual observations rather than universal expectations.

How Netra discusses results

Netra should distinguish subjective improvement from objective change, document the testing method, avoid implying regeneration of permanently lost tissue, and report non-response as well as response. Conventional treatment changes must remain under the prescribing ophthalmologist’s direction.

Questions to ask before interpreting an outcome

  • Was the diagnosis confirmed by the appropriate eye specialist?
  • Were baseline and follow-up tests performed under comparable conditions?
  • Was the change larger than expected measurement variability?
  • Were injections, medications, surgery or systemic health changes occurring at the same time?
  • Was the outcome sustained and clinically meaningful to the patient?

Current conclusion

NRT has a rationale for structured observational study as a complementary, multi-component program. The strength of any effectiveness claim should match the quality of the supporting evidence. Patients should receive transparent information about uncertainty, alternatives, costs and the importance of continued ophthalmic care.
