Who is eligible for the Vision Improvement Guarantee program?

• The Vision Improvement Guarantee program is only available for patients who are enrolled in the Comprehensive Treatment Package.
• Other treatment packages does not qualify for the Vision Improvement Guarantee program.
• Patient should undergo and successfully complete all therapies associated with the Comprehensive Treatment Package.
  

How is Vision Improvement determined after the treatment?

• Subjective and objective tests will be performed by board certified Ophthalmologist.
• Any marked improvement from these tests would be considered an improvement in vision.
  

PROGRAM EXCLUSIONS^*

What are the program exclusions?

• Patient who cannot complete all therapies associated with the Comprehensive Treatment Package for any reason whatsoever.
• Comprehensive Treatment Package only qualifies for the Vision Improvement Guarantee program.
