Central serous chorioretinopathy, commonly called CSCR, CSC, or central serous retinopathy, is a retinal condition in which fluid accumulates under the neurosensory retina, usually near the macula. The macula is the part of the retina responsible for sharp central vision, reading, facial recognition, contrast sensitivity, and detailed visual work. When subretinal fluid separates the photoreceptor layer from its normal support system, patients may experience blurred central vision, distortion, micropsia, muted color perception, dimness, reduced contrast, or a gray spot in the center of vision.
CSCR is often described as a leakage disorder, but that description is too narrow. Modern research increasingly places CSCR within the pachychoroid spectrum, a group of conditions characterized by choroidal thickening, dilated outer choroidal vessels, altered choroidal blood flow, vascular congestion, choroidal hyperpermeability, and stress on the retinal pigment epithelium. The retinal pigment epithelium, or RPE, normally helps regulate fluid movement, nutrient transport, waste clearance, and photoreceptor support. When the choroid is congested and the RPE barrier is stressed, fluid may accumulate beneath the retina.
